CDC developed a series of evaluation tools for use by state heart disease and stroke prevention programs, such as the WISEWOMAN program. The guides address writing SMART objectives, developing an evaluation plan, and other topics.

WISEWOMAN Program Evaluation Toolkit
The toolkit provides a framework for evaluating WISEWOMAN programs based on the CDC Framework for Program Evaluation in Public Health. The toolkit has four sections:
