These competitive grant programs expand and advance CDC’s National Center for Chronic Disease Prevention and Health Promotion (NCCDPHP) work with partners on education, outreach, and public awareness activities for a variety of chronic diseases that do not have dedicated CDC funding. Chronic diseases are defined broadly as conditions that last 1 year or more and require ongoing medical attention, or limit activities of daily living, or both.
Recipients focus on data dissemination, education, and public awareness activities that will support effective, evidence-based clinical and population-level approaches to improve health and health equity.
