DP14-1422
State and Local Public Health Actions to Prevent Obesity, Diabetes, and Heart Disease and Stroke
About the Program
This cooperative agreement built on and complemented activities initiated under 1305 and intensified work in state and large city health departments to prevent obesity, type 2 diabetes, heart disease, and stroke, and reduce health disparities. Each state sub-awarded half of the funds they received to support prevention activities in four to eight communities to reach high-burden priority populations with the largest disparities in high blood pressure and prediabetes. Under 1422, the state and city health departments conducted activities to address environmental, health care system, and community-clinical linkage strategies to prevent obesity, type 2 diabetes, heart disease, and stroke.
